Patient Ratio: 1:20
Case Load: Up to 25
REQUIRED:
- Active RN license
- Minimum 2 years of acute-care hospital Case Management experience
- Acute-care Case Management experience must be current within the last year
- Care coordination and discharge planning experience
- Epic experience
- Experience with HMOs, IPAs, etc.
- Experience with Advance Directives, POLST, and Protective Services processes
- Case Management / Utilization Review experience
- Admission criteria, benefits eligibility, needs assessment and DME coordination
- CMS, HIPAA, Joint Commission/Core Measures/National Patient Safety Goals knowledge
- Experience across acute-care settings such as ED, ICU, Med/Surg, Pediatrics, and PCU/IMC/Obs preferred
