Job Description :

•   Immediate start date required
•   Must be willing to float to the Laguna Facility as needed
•   Patient ratio: 1 Care Manager to 25 patients
•   Minimum 1- 2 years of acute medical Care Manager experience with care coordination required
•   Prior travel experience is mandatory; first-time travelers not accepted
•   Variable 5-day workweek; department operates 7 days/week
•   Days off will vary and may not be consecutive, though efforts will be made to provide consecutive days
•   Must hold a valid California state license (pending license accepted if active by start date)
•   No more than 7 days of requested time off (RTO) allowed
•   Work setting: Acute Care Facility
•   Dress code: Turquoise scrubs or business casual attire
