- Active California RN License — Pending accepted if licensed by start
- 2 years acute medical Care Manager experience
- Recent Care Coordination experience within the last 3 months
- Travel RN experience – HARDSTOP
- Epic experience – HARDSTOP
- Acute hospital Case Management experience
- Care Coordination & Discharge Planning
- DME, SNF & ambulance transport arrangements
- Experience with HMOs/IPAs
- Family meetings & treatment planning
- Strong psychosocial assessment and resource coordination skills
