Requirements

• Minimum 2 years of recent acute-care Medical Case Management experience
• Recent Care Coordination experience required, ideally within the last 3 months
• Travel experience required – first-time travelers are not accepted
• Active/valid California RN license required by start date
• Epic experience required
• Experience with assessment, care coordination, and discharge planning
• Ability to arrange DME, SNF placement, and ambulance transportation independently
• Experience with HMOs, IPAs, and family meetings preferred
• Strong knowledge of discharge planning and patient resource coordination
