Description

American Traveler is seeking an experienced RN Case Manager for an inpatient acute care hospital position requiring a CA RN license and a minimum of 5 years of RN Case Management experience.

Details

  • Acute care hospital setting, working within the Case Management/Utilization Review department
  • Responsibilities span care coordination and care transitions across the acute care patient experience
  • Collaborative care model involving physicians, utilization managers, medical social workers, and bedside RNs
  • Patient caseload ratio of 1:10
  • Day shift schedule, 5x8-hour shifts (8:00–16:30)
  • Every other weekend required

Requirements

  • Active CA RN license required
  • Current BLS certification (AHA or ARC) required
  • Minimum 5 years of RN Case Management experience required
  • Inpatient acute care Case Management (hospital) experience required
  • 2 supervisor references obtained within the past year required for consideration
  • 7-year work history verification required

Additional Information

  • Responsibilities include patient assessment, utilization management, care coordination, and care transition planning to ensure timely and appropriate patient progression through levels of care
  • Collaborates with an interdisciplinary team including physicians, social workers, payers, and community resources to develop and execute effective transition plans
  • Monitors length of stay, level of care requirements, and regulatory compliance with CMS and Joint Commission standards
  • Addresses complex clinical and social situations to prevent unnecessary admissions or readmissions, including for ED high utilizers
  • Local candidates are accepted; candidates must reside more than 50 miles from the facility to qualify for travel pay rates
  • Former employees may return without a required separation period; former manager's name will be needed
  • RTO is limited to 7 days per contract; only 1 holiday may be requested off per contract (eligible holidays: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day)
