SSN + DOB. AMIE PROFILE MUST INCLUDE CURRENT LICENSE(S) AND CERT(S) WITH VALID EXPIRATION DATES. RTO DATES MUST BE INCLUDED. 50 MILE RADIUS RULE PER DRIVER’S LICENSE ADDRESS.
