Home Health & Hospice CNA

Location: Marquette, MI
Schedule: Monday–Friday (4 days/week), 8:00 AM – 4:00 PM, with rotating Saturday coverage
Coverage Area: Up to a 45-mile radius from Marquette
Caseload: 7–10 patients per day

About the Role
One of our Connected Health Partners is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to provide care to patients receiving home health and hospice services. In this role, you will deliver hands-on personal care to patients in their homes while working under the direction of the RN Clinical Supervisor and Clinical Director.

This position is ideal for someone who is comfortable providing direct patient care and is passionate about supporting individuals and families during important stages of care.

Key Responsibilities

  • Provide personal care services to patients receiving hospice and home health services

  • Assist with activities of daily living including bathing, dressing, grooming, and shaving

  • Support patient mobility and assist with exercises as directed

  • Assist with meal preparation and feeding when needed

  • Perform light housekeeping tasks related to patient care

  • Observe and report changes in patient condition to the clinical team

  • Maintain accurate documentation within the EMR system

  • Travel within a 45-mile radius of Marquette to visit patients in their homes

Schedule & Caseload

  • Monday–Friday schedule covering 4 days per week from 8:00 AM – 4:00 PM

  • Rotating Saturday coverage

  • Typical caseload of 7–10 patients per day

Qualifications

  • CNA License (Required)

  • CPR/BLS Certification (Preferred)

  • Matrix EMR experience (Preferred)

  • Valid driver’s license and reliable transportation

  • Ability to travel within a 45-mile service area
