    Description

    We are seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a Skilled Nursing Facility (SNF). Working under the supervision of a licensed Occupational Therapist, the COTA will provide high-quality therapy services to help patients improve functional abilities, increase independence, and enhance their overall quality of life.

    Responsibilities
    • Implement individualized occupational therapy treatment plans developed by the supervising Occupational Therapist
    • Provide therapeutic activities and interventions to improve activities of daily living (ADLs), fine motor skills, upper extremity function, coordination, strength, balance, and overall functional performance
    • Monitor patient progress during treatment sessions and communicate changes or concerns to the supervising Occupational Therapist
    • Educate patients and caregivers on adaptive techniques, assistive devices, home exercise programs, and safety strategies
    • Maintain accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory standards
    • Collaborate with Occupational Therapists, physicians, nurses, physical therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team
    • Assist with discharge planning by providing updates on patient progress and recommendations for continued therapy
    • Maintain a safe, clean, and organized therapy environment
    • Deliver compassionate, patient-centered care focused on improving function, maximizing independence, and enhancing quality of life
    Requirements
    • Required: Graduate of an accredited Occupational Therapy Assistant program
    • Required: Active Certified Occupational Therapy Assistant (COTA) license in the state of practice or ability to obtain
    • Preferred: Previous experience in a Skilled Nursing Facility (SNF) or post-acute care setting
    • Strong communication, documentation, organizational, and interpersonal skills
    • Ability to work effectively as part of a multidisciplinary healthcare team
