We are seeking a compassionate and dedicated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a Skilled Nursing Facility (SNF). Working under the supervision of a licensed Occupational Therapist, the COTA will provide high-quality therapy services to help patients improve functional abilities, increase independence, and enhance their overall quality of life.

Responsibilities
Implement individualized occupational therapy treatment plans developed by the supervising Occupational Therapist
Provide therapeutic activities and interventions to improve activities of daily living (ADLs), fine motor skills, upper extremity function, coordination, strength, balance, and overall functional performance
Monitor patient progress during treatment sessions and communicate changes or concerns to the supervising Occupational Therapist
Educate patients and caregivers on adaptive techniques, assistive devices, home exercise programs, and safety strategies
Maintain accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory standards
Collaborate with Occupational Therapists, physicians, nurses, physical therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team
Assist with discharge planning by providing updates on patient progress and recommendations for continued therapy
Maintain a safe, clean, and organized therapy environment
Deliver compassionate, patient-centered care focused on improving function, maximizing independence, and enhancing quality of life
Requirements
Required: Graduate of an accredited Occupational Therapy Assistant program
Required: Active Certified Occupational Therapy Assistant (COTA) license in the state of practice or ability to obtain
Preferred: Previous experience in a Skilled Nursing Facility (SNF) or post-acute care setting
Strong communication, documentation, organizational, and interpersonal skills
Ability to work effectively as part of a multidisciplinary healthcare team
