Certified Occupational Therapy Assistant (COTA) – Long-Term Care

Contract | Days | Full-Time | Onsite | Generally five (5) days per week, likely including one weekend day
Key Responsibilities
- Provide rehabilitation services within a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
- Perform treatments and clinical services consistent with licensure, discipline, and scope of practice
- Develop and carry out individualized treatment plans in collaboration with the supervising therapist, physician, and interdisciplinary team
- Implement evidence-based therapeutic interventions to maximize resident function, independence, mobility, and quality of life
- Educate residents, families, and caregivers regarding treatment plans and patient progress
- Complete accurate, timely, and compliant documentation in accordance with facility, state, federal, and payer requirements
Required Qualifications
- Current, active State of Hawaii COTA license in good standing (must be obtained prior to assignment start date)
- Current Basic Life Support (BLS) certification
- Ability to work Hawaii State and Federal Holidays as required based on facility operations
- Willingness to float to other units or facilities based on census and staffing needs
Preferred Experience / Skills
- Minimum two (2) years of experience in a SNF or ICF setting
- Familiarity with EMR documentation platforms used in rehabilitation settings
