    Job Title

    Certified Occupational Therapy Assistant (COTA) – Long-Term Care

    Location

    Hilo, HI

    Schedule / Job Type

    Contract | Days | Full-Time | Onsite | Generally five (5) days per week, likely including one weekend day

    Key Responsibilities
    • Provide rehabilitation services within a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
    • Perform treatments and clinical services consistent with licensure, discipline, and scope of practice
    • Develop and carry out individualized treatment plans in collaboration with the supervising therapist, physician, and interdisciplinary team
    • Implement evidence-based therapeutic interventions to maximize resident function, independence, mobility, and quality of life
    • Educate residents, families, and caregivers regarding treatment plans and patient progress
    • Complete accurate, timely, and compliant documentation in accordance with facility, state, federal, and payer requirements
    Required Qualifications
    • Current, active State of Hawaii COTA license in good standing (must be obtained prior to assignment start date)
    • Current Basic Life Support (BLS) certification
    • Ability to work Hawaii State and Federal Holidays as required based on facility operations
    • Willingness to float to other units or facilities based on census and staffing needs
    Preferred Experience / Skills
    • Minimum two (2) years of experience in a SNF or ICF setting
    • Familiarity with EMR documentation platforms used in rehabilitation settings
