Connected Health Partners is seeking an experienced and compassionate Travel CNA for a 13-week assignment at one of our partner skilled nursing facilities near Rochester, MN. This is a great opportunity to expand your travel experience while making a meaningful impact on residents' quality of life.
Position Details:
- Provide direct patient care under the supervision of licensed nursing staff
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility
- Monitor and report changes in patient condition
- Maintain accurate documentation and support care plans
- Foster a supportive and respectful environment for residents and families
Requirements:
- Active CNA certification (Minnesota or compact eligibility preferred)
- Minimum 1 year of travel CNA experience required
- Previous experience in a skilled nursing or long-term care setting
- Current BLS certification
- Strong communication and teamwork skills
