Home Health Physical Therapist (PT) Job Responsibilities- Conduct comprehensive patient evaluations in the home health setting
- Develop individualized treatment plans focused on improving mobility, strength, balance, and independence
- Perform start of care assessments, recertifications, and routine home health visits
- Complete accurate OASIS documentation and maintain compliance with home health regulations
- Serve as a case manager coordinating care with physicians, nurses, therapists, and interdisciplinary teams
- Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions
- Monitor patient progress and modify treatment plans as needed
- Accurately document all patient interactions and treatment outcomes using Matrix EMR
- Deliver compassionate, patient-centered care while promoting functional independence and quality of life
Education & Certification Requirements- Doctorate or Masters degree in Physical Therapy from an accredited program
- Active Missouri Physical Therapist (PT) license required
- Current CPR/BLS certification preferred
- Home health experience highly preferred for travel assignments
- Strong experience with OASIS documentation, case management, start of care assessments, and recertifications required
- Experience with Matrix EMR preferred
- Excellent communication, organizational, and time management skills
- Ability to work independently in a home health environment
