Connected Health Care is partnering with a highly respected healthcare provider in Branson, Missouri to hire an experienced and compassionate Local Travel Home Health Physical Therapist (PT) for a 13-week contract assignment. This is an outstanding opportunity for a skilled PT seeking competitive weekly compensation, flexible travel opportunities, and the ability to make a direct impact on patients in the home health setting.
The ideal candidate will have prior home health experience and strong knowledge of OASIS documentation, case management, start of care assessments, recertifications, and routine home visits.

Home Health Physical Therapist (PT) Job Responsibilities
  • Conduct comprehensive patient evaluations in the home health setting
  • Develop individualized treatment plans focused on improving mobility, strength, balance, and independence
  • Perform start of care assessments, recertifications, and routine home health visits
  • Complete accurate OASIS documentation and maintain compliance with home health regulations
  • Serve as a case manager coordinating care with physicians, nurses, therapists, and interdisciplinary teams
  • Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions
  • Monitor patient progress and modify treatment plans as needed
  • Accurately document all patient interactions and treatment outcomes using Matrix EMR
  • Deliver compassionate, patient-centered care while promoting functional independence and quality of life
Education & Certification Requirements
  • Doctorate or Master’s degree in Physical Therapy from an accredited program
  • Active Missouri Physical Therapist (PT) license required
  • Current CPR/BLS certification preferred
  • Home health experience highly preferred for travel assignments
  • Strong experience with OASIS documentation, case management, start of care assessments, and recertifications required
  • Experience with Matrix EMR preferred
  • Excellent communication, organizational, and time management skills
  • Ability to work independently in a home health environment
