Job Requirements
Education Requirements
- Valid RN nursing degree (ADN or BSN) from an accredited program
Certifications Requirements
- Current and active RN license
- BLS (Basic Life Support) certification
- ACLS (Advanced Cardiac Life Support) certification (or willingness to obtain if required by facility)
Experience Requirements
- At least 1 year of RN experience in SNF, rehab, long-term care, or a closely related clinical setting
- Demonstrated experience caring for a medically diverse patient population in post-acute environments
- Experience working in an SNF/Rehab workflow including medication administration, documentation, and coordinated care
Other Requirements
- Strong weekend availability, including Friday, Saturday, and Sunday shifts
- Ability to work a multiple shift schedule (3-12) including 6a-6p and 6p-6a
- Excellent communication and documentation skills in a fast-paced care setting
Job Responsibilities
- Provide skilled nursing care to residents in a long-term care and rehabilitation environment during assigned weekend shifts
- Collaborate with interdisciplinary teams to support individualized care plans, patient goals, and discharge readiness
- Maintain accurate clinical documentation including assessments, medication administration records, and progress notes
- Execute medication administration and treatment plans in accordance with facility policies and provider orders
- Document and escalate patient changes promptly to ensure timely interventions and safe, effective care
