Certifications: Advanced Practice Certification preferred
Must-Have:
- Trained in the completion of Nutrition-focused Physical Exam (NFPE)
- Experience with enteral and parenteral nutrition
- Ability to read, write and speak English
- Strong computer skills and working knowledge of Microsoft Office products
- High level of interpersonal skills for effective communication
Description:
The role involves acting as a nutritional consultant and resource to members of the health care team.
Provides comprehensive nutritional care including assessment, Nutrition Diagnosis, implementation of care plans, counseling, education, and outcome evaluation for patients, families, and associates.
Coordinates various education programs and seminars on nutritional topics.
Key Responsibilities:
Assess the nutritional needs of patients by obtaining, verifying, and interpreting data.
Determine food and nutrition-related history, anthropometric measurements, biochemical data, medical tests, and other relevant information from the patient, family, medical record, and health care team.
Identify nutrition diagnosis to describe specific nutrition problems that can be resolved or improved through nutrition intervention.
Implement individualized nutrition interventions including food and nutrient delivery, nutrition education, and coordination of care based on patient needs.
Monitor and evaluate patient care outcomes to determine progress and whether expected goals are being met.
Ensure appropriate food consistencies and adequate nutritional intake through monitoring of food tolerances.
Collaborate and communicate effectively with health care providers, acting as a consultant and informing the team of significant diet-related information.
Educate patients and/or their significant others on nutrition and special diets based on physician’s orders.
Document nutrition services provided in the patient's medical record.
Develop and conduct in-service classes and community education sessions on health and nutrition topics.
Participate in special projects and committees related to nutritional care as needed.
Maintain and enhance professional expertise through educational and organizational activities.
Supervise dietetic interns/students, providing clinical teaching and mentoring.
Act as a liaison with culinary wellness departments to address various needs.
Demonstrate professional characteristics and ethics by adhering to policies and maintaining up-to-date knowledge and skills in the field.
Perform other related duties as required.
