Key Responsibilities
- Coordinate patient care across the continuum to ensure quality outcomes.
- Perform utilization review and support efficient resource management.
- Develop and implement safe, effective discharge plans.
- Collaborate with interdisciplinary teams to optimize patient care.
- Maintain accurate electronic documentation and demonstrate strong computer literacy.
Required Qualifications
- Active New York State Registered Nurse (RN) License.
- Primary Source Verification of RN License and Registration from the New York State Department of Education (NYSED).
- BLS Certification.
- PRI and Screen Certification (required at submission).
- Minimum 5 years of clinical experience as a Registered Nurse.
- Experience in Care Coordination, Utilization Review, and Discharge Planning.
- Associate Degree in Nursing (ASN/ADN) from an accredited nursing program.
