Job Summary:
Home Health and Hospice Registered Nurse (RN) position available in the specified area. The role involves performing initial assessments, developing and revising care plans, administering medications and treatments, and coordinating patient care within a 60-mile radius. The position requires on-call rotations during evenings, weekends, and holidays, with dayshift hours from 8:00 AM to 5:00 PM.

Job Details:

• Schedule: Dayshift, 8:00 AM - 5:00 PM, with on-call rotation during evenings, weekends, and holidays, including 24-hour coverage on weekends and holidays.
• Required Qualifications: Valid drivers license and auto insurance, BLS certification, a minimum of 1 year of home health and hospice experience, and active clinical involvement.
• Preferred Qualifications: None specified.
• Additional Information: Home Health and Hospice care experience required, serving adult, geriatric, and pediatric populations. The assignement includes initial patient assessment, care planning, medication administration, patient education, discharge planning, and coordination of equipment and services. Candidates must adhere to the radius rule of 50 miles and ensure no prior full-time, part-time, or PRN employment with associated health systems within the last year. RTO must be requested upfront, as additional requests may not be approved.
