**Home Health PT opportunity with THEMA Health Services in Prescott, AZ.** This position offers a **5x8 day schedule from 7:00 a.m.–3:00 p.m. with 40 hours guaranteed each week**, which is a really nice setup for Home Health. The position provides 1:1 patient care in the home and focuses on evaluations, individualized plans of care, gait and balance, transfers, strengthening, functional mobility, fall prevention, home safety and patient/caregiver education. Prescott is a beautiful mountain community in northern Arizona with a much milder climate than Phoenix.
