Key Responsibilities

• Provide standard physical therapy services to a home health patient population of all ages with multiple comorbidities
• Manage a caseload of approximately 4–5 patients per 8-hour day
• Complete all clinical documentation within 1 day of each patient visit
• Utilize HCHB (HomeCare HomeBase) EMR system for documentation and patient management
• Travel to patient homes; mileage is reimbursable per GSA rate
• Collaborate with support staff including COTAs and coordinate with a supervisor available 24/7

Required Qualifications

• Active California Physical Therapist (PT) license
• BLS certification required
• Valid driver's license and current auto insurance
• Prior home health experience required
• All required onboarding modules must be completed prior to start date — no exceptions

Preferred Experience / Skills

• Experience treating patients with complex, multiple comorbidities across all age groups
• Familiarity with HCHB (HomeCare HomeBase) EMR platform
• Strong time management and documentation skills in a field-based setting
