This Home Health Physical Therapist position offers the opportunity to practice in and around Griffin, Georgia, a welcoming community known for its small-town charm, revitalized historic downtown and easy access to metropolitan amenities. In this role, you will provide one-on-one, patient-centered physical therapy services in the home setting for adults and older adults recovering from illness, surgery, injury or managing chronic conditions. You will perform comprehensive evaluations, develop individualized plans of care, and deliver skilled interventions focused on improving mobility, strength, balance, gait and overall functional independence within each patient’s unique home environment. Your work will emphasize safety, fall prevention, and helping patients return to valued daily activities, whether that means navigating stairs, walking safely in the community, or completing household tasks with confidence. A typical day includes traveling to patient homes throughout Griffin and surrounding communities, conducting initial assessments and follow-up treatments, updating plans of care, and documenting your findings and interventions in the electronic medical record. You will coordinate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work and home health aides, ensuring that each patient’s care is cohesive, goal-directed and responsive to changes in condition. Communication with physicians and other providers is an important part of the role as you report progress, recommend modifications in treatment plans, and help guide discharge planning. Patient caseloads are primarily adult and geriatric, with diagnoses that commonly include post-acute orthopedic conditions, cardiopulmonary disease, neurologic impairments, general deconditioning and complex medical needs often associated with aging. Visit volume and scheduling are managed to support high-quality, individualized care and allow time for thorough documentation and patient education. You will frequently engage caregivers and family members in training sessions, teaching safe transfer techniques, home exercise programs and strategies for modifying the home environment to reduce fall risk and maximize independence. The practice environment is designed to support both clinical excellence and professional growth.
