Contract | Days | Monday–Friday 8:00 AM–5:00 PM (flexible); every 3rd weekend required; minor holidays required, major holidays as needed

Key Responsibilities

• Provide physical therapy services to patients in a home-based care setting within an assigned geographical region
• Treat a caseload that includes orthopedic conditions, strokes, joint disorders, and spinal injuries
• Complete and maintain OASIS documentation in accordance with government requirements
• Utilize Cerner EMR for all clinical documentation
• Collaborate with interdisciplinary team members including OT, Speech, and support staff
• Adhere to professional dress standards: business attire with white lab coat (no jeans)

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• BLS certification required
• Minimum of one (1) year of experience as a Physical Therapist
• Experience with OASIS documentation (government-mandated assessment tool)

Preferred Experience / Skills

• Prior experience in a home health or home-based care setting
• Familiarity with Cerner EMR
• Experience treating orthopedic, neurological, and spinal injury patient populations
