This Home Health Physical Therapist opportunity is based in Hackensack, New Jersey, a lively and well-connected community in Bergen County. In this role, you will deliver skilled physical therapy services to patients in their homes across Hackensack and nearby communities. As a Home Health Physical Therapist, your typical day will include traveling to patient residences, performing thorough evaluations, and developing individualized plans of care. You will focus on improving mobility, strength, balance, and functional independence while emphasizing safety in the home environment. Patients may be recovering from surgery, living with chronic conditions, transitioning from hospital or inpatient rehab, or managing neurological or cardiopulmonary diagnoses. Your responsibilities may include: Conducting initial and follow-up assessments in patient homes to identify functional limitations, environmental risks, and rehabilitation potential Designing and implementing evidence-based treatment plans tailored to each patient’s goals, medical status, and home setting Providing therapeutic exercises, gait and transfer training, balance and fall-prevention programs, and instruction in use of assistive devices Educating patients, families, and caregivers on home exercise programs, energy conservation strategies, safe mobility techniques, and modifications that support independence Collaborating with an interdisciplinary team, including nursing, occupational therapy, speech therapy, social work, and other professionals, to ensure coordinated, patient-centered care Documenting all care provided using electronic medical records, maintaining compliance with regulatory and agency standards, and submitting documentation in a timely manner Monitoring patient progress, updating plans of care, and communicating changes in status to the broader care team Contributing to discharge planning and helping patients transition to community-based resources or outpatient services when appropriate Typical caseloads and patient ratios in home health are structured to allow for meaningful one-on-one time with each individual. You will generally see a set number of patients per day based on agency expectations and travel distances, with each visit focused on personalized, hands-on care. Because you are working in patients’ homes rather than a traditional facility, you will not be assigned to inpatient bed counts; instead, your practice will revolve around scheduled home visits, documentation periods, and coordination calls. Shifts commonly follow daytime hours with some flexibility to accommodate patient needs and route planning, allowing for a balanced schedule and autonomy in organizing your day.
