Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience to provide skilled care to patients recovering at home in the Mokena/Frankfort area.

Details

• Home health setting covering the Mokena/Frankfort territory
• 8-hour day shifts
• Provides age-appropriate care for patients ranging from pediatric to geriatric
• Mileage reimbursed at the standard IRS rate

Requirements

• Minimum 1 year of Home Health PT experience required
• BLS certification required
• IL state license required if applicable
• Valid driver's license and car insurance required
• COVID vaccination required; declinations will not be approved
• OASIS and start of care experience strongly preferred

Additional Information

• Evaluate and treat patients to improve mobility and independent function while developing individualized care plans
• Coordinate and communicate with patients, families, physicians, and staff in line with hospital values
• Complete timely documentation of all patient care activities in the electronic medical record
• Understand admission processes, treatment costs, coding, and appropriate billing
• Ability to transfer and position patients as needed
• Local candidates will be considered if within the designated territory
• No more than 5 days of RTO can be requested during the assignment
