Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience for a day-shift assignment in the Medford, OR area.

Details

• Home health setting serving patients in Medford, OR and surrounding areas
• Day shift schedule, 8-hour shifts, 5x8 (8:00 AM-4:30 PM)
• No 4x10 or 3x12 schedules accommodated
• 1:1 patient ratio, visiting up to 6 patients per day with a standard of 30 productivity points
• No weekend shifts required
• No on-call requirements
• No floating between like units required
• Uses EPIC and Oasis for charting
• Travel between patient visits is generally minimized, with occasional trips of 50+ miles

Requirements

• Minimum 1 year of skilled home health experience required
• BLS certification required
• Oasis charting experience required
• EPIC EMR experience required
• Must have reliable personal transportation for patient visits
• Primary series COVID-19 vaccination plus one booster required
• License required if applicable

Additional Information

• Responsibilities include home exercise program development and patient/family education
• Clinical focus includes orthopedic conditions (post-op joint replacements, spinal surgery, low back pain) and neurologic conditions (stroke, TBI, Parkinson's, MS)
• Scrubs of any color are accepted
• Parking is free and mileage is reimbursed at the IRS standard rate
• First-time travelers are not accepted for this position
• Local candidates accepted within a 50-75 mile radius at the same pay rate
• Former permanent staff must wait at least 3 months before returning as a traveler and must live more than 50 miles from the facility
• RTO requests should be confirmed with the hiring manager, as minimal time off is accommodated
