Your typical day includes traveling between patient residences, completing comprehensive evaluations, designing individualized care plans, and delivering hands-on interventions focused on functional mobility, strength, balance, and fall prevention. You will assess home environments and recommend adaptive strategies or equipment to improve safety and independence. You will collaborate closely with a multidisciplinary home health team that may include nursing, occupational therapy, speech therapy, social work, and home health aides. Communication is primarily through EMR, phone, and team meetings, allowing you to maintain autonomy in the field while remaining connected to supportive colleagues. Caseloads are structured to allow time for travel, documentation, and thorough patient-centered care, with visit volumes calibrated to maintain high-quality interventions. Documentation is completed in an electronic medical record system, and you will be responsible for timely, accurate notes, updating care plans, and communicating progress to the broader care team. You will educate patients and families on home exercise programs, proper use of assistive devices, and strategies to manage chronic conditions and prevent re-hospitalization. Shifts are generally daytime hours on weekdays, with some flexibility based on patient needs and scheduling preferences. Weekend or evening coverage may be available or occasionally requested, offering opportunities to adjust your schedule as needed. Patient ratios are structured around individual visits rather than unit-based staffing, giving you focused one-on-one time with each patient.
