This Home Health Physical Therapist role is based in Norwood, Massachusetts, a welcoming suburban community known for its friendly neighborhoods, tree-lined streets, and vibrant town center. In this position, you will provide skilled physical therapy services to patients in their homes throughout Norwood and surrounding communities. Each day offers a meaningful blend of autonomy and collaboration as you travel between patient visits, perform comprehensive evaluations, and design individualized treatment plans that help patients maintain safety, independence, and quality of life. Typical patients include adults and older adults recovering from surgery or hospitalization, managing chronic conditions, or needing support to remain safely at home. A typical day may include conducting initial evaluations; developing and updating care plans; implementing therapeutic exercises; gait and balance training; fall-prevention strategies; home safety assessments; and education on the use of assistive devices and durable medical equipment. You will also focus on teaching patients and caregivers home exercise programs that are practical and sustainable in real-world home environments. Visit volumes are structured to allow time for thorough, high-quality care and complete documentation, with caseloads and scheduling designed to support safe, effective practice. You will work closely with an interdisciplinary team, participating in regular communication with nursing and other therapy disciplines to coordinate care, monitor progress, and adjust treatment plans.
