Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience and proficiency in OASIS documentation.

Details

• Home health setting
• Day shift schedule, 5x8-hour shifts from 8:00 AM to 4:30 PM
• 13-week assignment length
• Floating may be requested based on facility needs
• Requires proficiency with OASIS documentation
• Epic EMR experience required

Requirements

• Minimum 1 year of home health experience required
• OASIS experience required
• BLS certification required
• Manual therapy certification preferred
• Epic charting experience required
• License required if applicable
• Specialty experience within the last year required
• Skills in home exercise programs, adaptive devices, gait training, and assistive devices
• Experience with orthopedic, cardiopulmonary, and neurologic patient populations preferred
• Familiarity with equipment such as gait belts, walkers, hospital beds, and slings

Additional Information

• Responsibilities include patient assessment, evaluation, treatment planning, and discharge documentation
• Duties may include ordering supplies and scheduling patient appointments
• Business casual attire or solid color scrubs permitted
• Local candidates accepted at the same pay rate
• Candidates should keep requested time off to a minimum during the assignment, ideally no more than 7 days total
• Previous employees must have a gap of at least 6 months since perm employment before being eligible for travel assignments
