Responsibilities
- Evaluate patients in their homes and develop individualized treatment plans
- Provide skilled physical therapy services to improve strength, mobility, balance, and functional independence
- Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
- Monitor patient progress and adjust treatment plans as appropriate
- Coordinate care with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team
- Complete timely and accurate electronic documentation
- Ensure compliance with home health regulations and agency policies
Qualifications
- Doctor of Physical Therapy (DPT) or Bachelor's/Master's Degree in Physical Therapy from an accredited program
- Current California Physical Therapist License (or eligibility)
- Current BLS Certification
- Home Health experience preferred
- Valid driver's license and reliable transportation
- Excellent communication, organization, and time management skills
- Ability to work independently while managing a flexible daily schedule
