Description

American Traveler is hiring a Home Health Psych RN for a PACT program role requiring at least one year of psychiatric experience and a reliable personal vehicle.

Details

• Hybrid position combining in-home patient visits with on-site work at a Mental Health and Wellness Center as needed
• Supports adults in the PACT (Program for Assertive Community Treatment) with an average program tenure of about 4 years
• Primary diagnoses include Schizophrenia and Schizoaffective Disorder, with secondary diagnoses spanning mood, anxiety, trauma, neurodevelopmental, psychotic, and personality disorders
• Caseload of approximately 10 patients, with program total of 87 patients
• 8-hour day shifts, Monday through Friday, with choice of 0800, 1000, or 1130 start time
• No on-call responsibilities
• Laptop and cell phone provided for fieldwork
• May be required to work one holiday during the contract

Requirements

• Active WA or compact RN license required
• BLS certification required
• CPI or MOAB certification highly preferred
• Minimum 1 year of psychiatric nursing experience required
• Personal vehicle, valid driver's license, and auto insurance required
• Experience with long-acting antipsychotic injections, prior authorizations, and refill coordination
• Skilled in psychotropic medication management and side effect assessment (EPS, tardive dyskinesia, metabolic concerns)
• Experience with AIMS assessments, physical health screening, and crisis assessment including suicide and violence risk evaluation
• Substance use assessment and intervention experience required
• Strong independent decision-making, safety awareness, and risk assessment skills in the field
• Experience conducting home visits and outreach with difficult-to-reach clients
• Proficiency with EPIC EMR and Microsoft Outlook required

• Conduct home visits and outreach alongside on-site clinic work as needed
• Requires flexibility and adaptability in non-traditional, community-based care environments
• Collaborate with community resources to support patient care navigation
