Job Summary:
Home Healthcare Nurse in Meridian, Idaho. This position involves providing in-home patient care, with a focus on assessing Medicares homebound criteria, determining the primary skill for each visit, and executing comprehensive OASIS documentation. The role requires strong clinical judgment, attention to detail, and the ability to work within a tight geographical area to optimize visit efficiency.
Job Details:
- Location: Meridian, Idaho
- Shift Schedule: Rotating shifts, primarily Monday through Friday, 8:30 AM to 5:00 PM, with weekend on-call rotation
- Certification Requirements: BLS certification from AHA or ARC
- Experience Requirements: At least 5 years of home healthcare experience, with a focus on home health assessments and OASIS documentation
- Experience Preferred: A minimum of 2 years, CRM experience (HCHB or other), and familiarity with EPIC Remote Client for documentation
- Schedule: 8-hour shifts, a minimum of 40 hours per week
- Additional Information:
- Home base during orientation, then flexible to start and end days at home
- Travel between patient visits kept within 15-20 minute drive time
- Typically 5-6 routine visits per day, with no overtime available
- Patient visit schedule managed proactively in case of cancellations
- Laptop and phone issued; transitioning to Rover platform
- No on-site backfill required for patient appointments
