Job Summary:
Home Healthcare Registered Nurse (RN) position in Lexington, Kentucky. This role involves providing home health services to adult patients, including start of care, recertifications, and daily visits. The position requires managing a caseload of approximately 30 patients and offers scheduled shifts that include weekends and holidays on a rotation basis.
Job Details:
- Schedule: 5-8 hour shifts, Monday through Friday, with weekend and holiday rotations.
- Required Qualifications: Valid Kentucky nursing license; BLS Certification; minimum of 1 year of experience on a Medsurg floor in a hospital setting; must hold licensure in the state of work.
- Preferred Qualifications: Home care/home health experience; home vents management experience is not required.
- Patient Caseload: Approximately 30 adult patients daily, with a case point system of 6.5 points per day.
- Coverage Area: Home health services only.
- Resource Support: Clinical manager or educator available 24/7 for emergencies or during business hours.
- Additional Information: Orientation includes two weeks with a preceptor and in visits with a work coach. Equipment provided includes phone and tablet.
- Charting System: Homecare Home Base (HCHB).
- Shift Notes: 5-8 hour shifts, Monday through Friday, with rotational weekend and holiday work.
- Certification: Must have BLS Certification.
