Job Summary:
The organization is seeking a Home Health Registered Nurse (RN) located in Massachusetts. The role involves assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families within a community health setting. The nurse will perform admission processes and case management responsibilities in accordance with organizational policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communication.

Job Details:

  • Schedule: Day shifts, 8 hours per shift, with a minimum of 40 hours per week
  • Required Qualifications: 1 year of certified home health care management experience, 1 year of case management experience, ability to travel to assigned cases, and a minimum of 1 year of relevant work history
  • Preferred Qualifications: Experience in hospice, private care, or working solely as a case manager without home health care agency experience is not considered relevant for the requirements
  • Additional Information: Weekend rotations are scheduled once every 6 weeks. Candidates must be willing to travel to cases within the specified geographic area.
