Description

American Traveler is seeking a Clinical Care RN for a PACE clinic and home health setting, requiring an active MI RN license and geriatric care experience.

Details

• Combined clinic and home-based care setting serving frail, elderly, and long-term care participants
• Schedule is Monday through Friday, 8:00am to 4:30pm
• On-call and holiday rotation required
• Provides direct nursing care including wound care, lab draws, and nurse assessments during home visits
• Collaborates with Primary Care Providers and coordinates monitoring of acute and chronic conditions

Requirements

• Active MI RN license required
• Associate's degree required; Bachelor's degree in Nursing preferred
• RN diploma or degree from an accredited School of Nursing or University
• Minimum of one year experience working with frail, elderly, or long-term care population
• One year of home care experience preferred
• One year of wound care experience preferred
• Current CPR certification by AHA required, or willingness to obtain
• Must have reliable transportation, a valid MI driver's license, and car insurance
• Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B

• Works independently applying advanced nursing knowledge to manage complex clinical situations
• Triages emergent situations and administers treatments, procedures, and medications as prescribed
• Maintains a visible presence within the center and communicates regularly with staff, participants, and caregivers
• May be required to work beyond normal hours, including weekends and holidays, as necessary
