Description

American Traveler is hiring a Home Health RN with at least 1 year of experience and HCHB charting system proficiency for a Monday-Friday day shift assignment.

Details

• Home health setting providing care to adult patients discharged from hospitals, MD offices, and SNFs
• Caseload requires a minimum of 6 points per day
• Patient ratio of approximately 1:6
• Day shift schedule, 5x8 hours, Monday through Friday, 8:00 AM to 5:00 PM
• No weekend or on-call requirements
• No overtime

Requirements

• Active Maryland or Compact RN license required in hand at time of consideration
• BLS certification required
• Minimum of 1 year of home health nursing experience required
• Prior travel nursing experience required
• HomeCare HomeBase (HCHB) charting experience required
• Home care experience preferred
• Community hospital experience preferred
• Previous charge nurse experience preferred
• Two supervisor references required for consideration

Additional Information

• Provide comprehensive home health nursing care including admission assessments, care planning, and discharge coordination
• Skilled in wound care, medication administration, catheter management, and chronic disease monitoring
• Perform OASIS documentation and coordinate services across multiple specialty areas including respiratory, gastrointestinal, and oncology care
• Candidates must be willing to float within scope of practice
• Facility is tobacco-free and requires candidates to be nicotine-free; a positive nicotine screen will result in cancellation
• Former employees must be separated from the facility for at least 1 year
