Description
American Traveler is hiring an experienced RN for a Home Health position in Kāne'ohe, HI, requiring Medicare-certified home health agency experience and proficiency with Homecare Home Base and OASIS.
Details
- Home health setting in Kāne'ohe, HI
- Day shift schedule with 8-hour shifts, 40 hours per week
- 13-week contract with Tuesday start dates
- Rotating weekends required, approximately 2-3 per month
- On-call required approximately 6 shifts per month, including weekends
- Patient population includes complex patients, frequent ED utilizers, chronic pain and substance abuse patients, and homeless patients
- Uses Homecare Home Base EMR; experience required
- OASIS documentation experience required
Requirements
- Active HI RN license required
- Minimum 1 year of experience in a Medicare-certified home health agency
- Proficiency with Homecare Home Base and OASIS required
- Active, clear driver's license and current auto insurance required
- COVID and Flu vaccinations required; declinations are not accepted
- Bachelor's degree preferred
- Case management experience preferred
Additional Information
- Serves as a liaison between the patient, family, nursing staff, physicians, and the multidisciplinary care team to optimize patient outcomes
- Leads coordination of patient care across disciplines, monitoring appropriateness and timeliness of care delivery
- Ensures the interdisciplinary care plan aligns with the patient's clinical course, continuing care needs, and covered services
- Collaborates with acute care case managers and coordinates patient transfers to tertiary centers, including continuity of patient information
- Manages frequent ED visitors through focus study reviews of previous and current admissions
- Collects and provides oversight of data required for regulatory and accreditation compliance
- Mileage is reimbursed at $0.75 per mile
- No more than 5 days of RTO may be requested for this assignment
