Description

American Traveler is hiring a Home Health RN with MDS/OASIS assessment experience and an active MA RN license for an assessment-focused assignment.

Details

  • Home Health setting focused primarily on patient assessments
  • RN completes assessments at the patient's home and/or office
  • Works alongside a care partner who typically has an established history with the patient, except for new admits
  • Day shift schedule (5x8), with hours varying between approximately 8:00-16:00 and 9:00-17:00 depending on the day
  • 40 hours per week guaranteed with flexibility required for varying start/end times
  • No hands-on patient care; role focuses on assessments for insurance/programs and developing care plans
  • May include additional duties such as medication reconciliation
  • Floating required to all CHL locations within a 38-mile radius
  • Union facility with no special scheduling accommodations; must be able to work all 7 days as needed

Requirements

  • Active MA RN license required (permanent, in hand)
  • BLS certification required
  • Minimum 1 year of recent RN experience required
  • Experience with MDS or OASIS assessments required
  • LTC or similar experience accepted if documented assessment experience is included on profile
  • Reliable transportation required, as RN may drive independently or carpool with care partner
  • Psychiatric experience helpful but not required
  • Must complete and pass a Tele test (80% passing score, 1 attempt) during orientation
  • Must complete a Pass/Fail Medication exam during orientation
  • Charting system experience required (any EMR)

Additional Information

  • Responsibilities include conducting home/office-based assessments, completing MDS/OASIS documentation, and developing care plans
  • Preference given to candidates with no requested time off (RTO); up to 14 days may be submitted for review, with no RTO allowed in the first week
  • Local candidates accepted at the same bill rate within a 200-mile radius
  • Returning staff must have a 12-month separation from the health system
  • Multiple study materials and modules provided during compliance and orientation process
  • Orientation includes pre-start modules (1-2 hours) and additional modules during orientation, completed annually
  • First-time travelers are not preferred for this assignment
