Registered Nurse – Home Health
Greensboro, NC
⏳ 13 Weeks Contract (Extension Possible)
Start: 08/24/2026
Shift: Day – Monday through Friday (8:00 AM – 5:00 PM)
• No inpatient care – community-based Home Health position
• HCHB (Homecare Homebase) documentation system utilized
Position Details:
• Home Health Nursing
• Pediatric patients with on-the-job training provided
• Case management role with an assigned patient caseload
• Community-based travel throughout the service area
Requirements:
• Active RN License
• Minimum 1 year of Home Health case management experience required
• Homecare Homebase (HCHB) experience required
• Previous travel nursing experience required
• Must be willing to manage a patient caseload
• Valid driver's license, reliable transportation, and auto insurance required
⭐ Preferred:
• Pediatric Home Health experience (training provided if needed)
• Strong communication and organizational skills
Required Skills:
• Home Health case management
• Comprehensive patient assessment
• Care planning and coordination
• Medication management and patient education
• OASIS documentation (if applicable)
• HCHB documentation proficiency
• Time management and independent clinical decision-making
• Collaboration with physicians and interdisciplinary care teams
Responsibilities:
• Provide skilled nursing care to patients in their homes
• Manage an assigned caseload and coordinate individualized care plans
• Perform comprehensive patient assessments and ongoing evaluations
• Educate patients and families on disease management, medications, and home care
• Complete accurate documentation using Homecare Homebase (HCHB)
• Travel throughout the assigned service area using a personal vehicle
• Provide care to pediatric patients following orientation and training
• Collaborate with physicians and interdisciplinary care teams to deliver safe, high-quality, patient-centered care
