Description

American Traveler is seeking an experienced Home Health RN for a day-shift travel contract in the West Portland, OR area, requiring a valid OR RN license and at least 1 year of skilled home health experience.

Details

  • Home health setting serving West Portland, OR and surrounding areas
  • Caseload of up to 6 patients per day with a standard productivity expectation of 30 points
  • Adult home health patient population including wound care, IV therapy, medication management, and GI/ostomy care
  • Day shift schedule, 5x8 hours (8:00–16:30); 4x10 and 3x12 schedules cannot be accommodated
  • No weekend or on-call requirements
  • Charting via Epic Remote Client for Home Health (on laptop) and Rover (phone app); OASIS documentation required
  • Floating between facilities within the state of Oregon is expected; refusal to float may result in assignment termination

Requirements

  • Active Oregon (OR) RN license required
  • BLS certification required
  • Minimum 1 year of skilled home health experience required
  • OASIS experience required
  • Epic EMR experience required
  • Reliable personal transportation required, as daily travel to patient homes is necessary
  • Full SSN and DOB required for consideration
  • Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting
  • Primary COVID-19 vaccination series plus one booster required

Additional Information

  • Daily driving is required; the system groups clients to minimize drive time, though occasional routes may total up to 60 miles or 60–90 minutes of drive time per day
  • Responsibilities include start of care, routine visits, recertification, resumption of care, admission assessments, and discharge documentation
  • Clinical skills include wound care (including VAC), IV infusions, PCA pump management, subcutaneous/IM/intradermal medication administration, ostomy care, and G-tube site care
  • Business casual attire required; scrubs are allowed (no specific color) but jeans are not permitted
  • First-time travelers are welcome to apply
  • Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate
  • Former permanent employees of this health system must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the submitting facility
  • RTO is expected to be minimal; any time-off requests should be disclosed upfront
