Requirements
- Active Vermont RN License (Required)
- Minimum Home Health experience preferred
- OASIS experience required
- Current BLS/CPR Certification
- Acute Care nursing experience preferred
- Experience with Homecare Homebase (HCHB) preferred
- Valid driver's license with reliable, insured vehicle
- Strong assessment, documentation, communication, and case management skills
