## Join Our Team as a Passionate RN in Home Health! **Job Description:** - Position: RN - Home Health (Full-Time) - Schedule: Monday to Friday, with weekend rotation approximately every 4th week - Join a dynamic, community-focused, not-for-profit organization committed to enhancing the well-being of our neighbors in central Vermont. - Deliver exceptional home health and hospice care to individuals of diverse backgrounds, ensuring high-quality services are accessible to all. - Provide holistic care, promoting health and well-being at every stage of life. **Key Responsibilities:** - Deliver individualized, goal-oriented nursing care in accordance with a physician’s orders. - Act as the Case Manager when designated as the lead discipline for patient cases. **Requirements:** - COVID Vaccination Required - No exemptions. - OASIS experience is a must. - Previous home health experience is required. - Experience as a home care traveler with proficiency in OASIS is preferred, especially for case management tasks. - Homecare Home Base experience is preferred. - Minimum of 1 year of nursing experience. - Certifications: BLS/BCLS required. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
