Description

American Traveler is hiring a Home Health RN Float for a 13-week assignment requiring MA RN licensure and certified home health case management experience.

Details

• Home health setting covering Chelsea, East Boston, Revere, and Winthrop
• Float role requiring travel to patient cases as assigned
• Day shift schedule from 08:00-16:30
• Weekend rotation once every 6 weeks
• 13-week assignment length
• Uses Epic for charting

Requirements

• Active MA RN license required (no conditional or pending licenses accepted)
• Current AHA-issued BLS certification required
• Minimum 1 year of experience in a certified home health agency required
• Minimum 1 year of case management experience within a certified home health agency required
• Hospice, private care, or standalone case management experience does not meet qualification requirements
• Primary source license verification required through NURSYS and BON within 30 days of start

• Assess patient conditions and implement individualized care plans
• Provide ongoing education and support to patients and families
• Perform admission processes and case management duties per agency policy and standards
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Flu vaccination required with no religious or medical exemptions allowed
• Local candidates accepted if permanent residence is within 60 miles of Massachusetts General Hospital in Boston
