Description

American Traveler is hiring a Home Health RN Float requiring an active MA RN license and 1 year of certified home health agency case management experience.

Details

• Home health setting covering Boston-area neighborhoods including Back Bay, Beacon Hill, Cambridge, Charlestown, and surrounding communities
• 13-week assignment with day shift hours from 08:00-16:30
• Weekend rotation required once every 6 weeks
• Requires ability to travel to patient cases as assigned
• Uses Epic for charting

Requirements

• Active MA RN license required; conditional or pending licenses not accepted
• Both NURSYS and BON verification required for nursing license
• AHA-issued certifications only
• 1 year of experience within a certified home health agency required
• 1 year of case management experience required
• Hospice, private care, or standalone case management experience does not satisfy the experience requirement
• Flu vaccine required with no religious or medical exemptions allowed

• Assess patient conditions, implement care plans, and provide ongoing education and support to patients and families
• Perform admission processes and case management duties in accordance with home care policies and standards
• Maintain thorough and timely electronic documentation and interdisciplinary communication
• Deliver care in alignment with organizational philosophy and community health practice standards
• Local candidates permitted if permanent residence is within 60 miles of Boston, at a reduced rate
• Start date contingent on credentialing clearance; delayed clearance may result in a rescheduled start date
