GLC On-The-Go is seeking a travel nurse RN Case Manager, Home Health for a travel nursing job in Barre, Vermont.
As a Home Care RN specializing in Hospice, you will join a community-focused, not-for-profit organization dedicated to enhancing the well-being of individuals in Central Vermont. We deliver exceptional home health and hospice care to individuals of all backgrounds, ensuring access to high-quality services regardless of financial circumstances. This position is responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician's order. In addition, this position serves as Case Manager in cases in which this position is the lead discipline.
