As a Home Health SLP in this role, you will provide comprehensive speech-language pathology services to patients in their homes throughout Covington and surrounding communities. You will conduct detailed evaluations of speech, language, voice, cognitive-communication, and swallowing function, then develop and implement individualized treatment plans designed to maximize independence and quality of life. Your work will focus primarily on adult and geriatric patients who may be recovering from stroke, surgery, or other medical events, or managing progressive conditions that affect communication and swallowing. Your typical day may include traveling to multiple patient homes, performing assessments, delivering interventions, and documenting care using an electronic medical record system. You will collaborate closely with nurses, physical and occupational therapists, medical providers, and social workers to ensure integrated, patient-centered care. Responsibilities include educating patients and caregivers on communication strategies, safety precautions, swallowing techniques, and home exercise programs, as well as monitoring progress and adjusting treatment plans as needed. The home health environment allows for meaningful one-on-one care in the patient’s own surroundings, enabling you to tailor therapy to real-life situations and functional goals. Visit volumes and patient caseloads are structured to support thorough, high-quality care and professional autonomy. You may see a range of patients with diagnoses including stroke, traumatic brain injury, dementia, Parkinson’s disease, and other neurologic or medical conditions, as well as those needing short-term rehabilitation following hospitalization. Shifts typically follow daytime hours with some flexibility based on patient needs and scheduling, allowing you to plan your day and route efficiently around Covington and nearby neighborhoods. While this position does not involve traditional inpatient bed counts, you will be part of a broader continuum of care, often transitioning patients from hospital or rehabilitation settings to home-based services and supporting their recovery and independence.
