Productivity expectations: 6 visits per day/ 30 points a week

Coverage Areas: Shelby/Fayette/Tipton
Shift schedule (days, start & end times): 8-4:30 M-F
Will Mileage be reimbursed between visits, from first patient to last patient (Y/N)? Y

Must have qualifications/experience: Hospice experience and HCHB a must
