Job Title: Clinical Care Team Social Worker or Social Counselor Profession: Social Worker 2 Specialty: Medical Duration: 46 Shift: Regular Shift Hours per Shift: 8:00 AM - 5:00 PM Experience: One year of full-time professional social work or social work related experience; or qualifying graduate coursework may be substituted for experience. License: Current master of social work certificate or independent practitioner of social work license as specified in state requirements. Certifications: None specified Must-Have: Graduation from an accredited college or university with a degree in social work or a related field. Description: The team member will support primary care clinics in the region. The social worker or social counselor will work in their assigned clinic(s) and collaborate with clinic staff to address patient needs. They will meet one-on-one with patients to identify and connect them with community partners offering resources addressing social determinants of health. There will be an expectation to attend regional office meetings related to clinical care and processes. Travel within the region may be expected as needed. Under general supervision, they will be responsible for professional social work or social counseling work and/or service coordination of average difficulty. The main tasks include performing tasks consistent with social work or social counseling such as referral and coordination of services. They will identify risk factors, assess, and address relevant patient needs such as educational, medical, psychosocial, and financial. The Clinical Care Team will take referrals from primary care providers. They will work with the primary care team to accomplish the following tasks: Social support navigation for social determinants of health, including food insecurity and housing insecurity. They will compile and maintain a resource list for social determinant of health resources, including eligibility criteria and contact information. Collaboration with primary care team members is essential. They will provide in-person or remote social needs screening and assessment for patients referred by nurses or providers. They will coordinate or inform patients about social services resources such as housing, clothing, food, and mental health services. Collaboration with other social workers to identify patient and community resources is important. Case management activities will include: Working with support facilities for discharge planning, follow-up, and education. Assisting with obtaining patient records. Securing needed medical equipment through community partners. Conducting follow-up on care plans. Identifying patients who are lost to follow-up or overdue for care and assisting them in returning to care. Specialty referral navigation may also be required. Scheduling, coordinating, and tracking referrals will be part of the responsibilities. They will assist with obtaining patient records from specialists and compile and maintain a resource list for specialty referrals. Assisting patients to locate and access low-cost prescription options such as patient assistance programs is crucial. Application assistance for programs and serving as a patient-provider liaison will be expected. The team member may help with other regional primary care-based initiatives with a social work component. Documentation in patient records is required along with updating consults and tagging providers or clinical staff as necessary. Providing patient education or finding appropriate educational resources is part of the role. Expectations also include: Completing onboarding and training. Participating in clinical meetings and provider meetings as requested. Attending community meetings to build relationships with agencies and promote service offerings. Identifying barriers to care or assistance experienced by patients and seeking ways to address them. Necessary tools and equipment will include personal computers, telephones, fax machines, printers, scanners, copy machines, calculators, and personal vehicles.
