Key Responsibilities of the Travel Social Worker- Conduct discharge planning and assessments
- Perform SBIRT/Substance assessments
- Adapt to various unit assignments as needed
Requirements of the Travel Social Worker- Minimum 1-year experience as a Social Worker, preferably in an acute care setting
- Active CO licensure
- Experience in discharge planning and ED settings is advantageous
- Knowledge of palliative care and behavioral health with substance use assessments is preferred
