- Weekly Pay:$2030 to $2230
Responsibilities
- Conduct comprehensive psychosocial assessments to identify patients' emotional, social, behavioral, and environmental needs
- Develop individualized treatment and care plans based on patient assessments
- Provide individual, family, and group counseling to support emotional well-being and recovery
- Offer crisis intervention and emotional support for patients and their families
- Coordinate discharge planning and connect patients with appropriate community resources and support services
- Collaborate with physicians, nurses, case managers, therapists, and interdisciplinary healthcare professionals to ensure coordinated patient care
- Advocate for patients to ensure access to healthcare services and community programs
- Educate patients and families regarding available resources, coping strategies, and ongoing support services
- Maintain accurate, timely, and confidential documentation in the electronic medical record (EMR)
- Participate in multidisciplinary care meetings and contribute to patient care planning
- Ensure compliance with facility policies, regulatory standards, and ethical social work practices
Requirements
- Active Licensed Clinical Social Worker (LCSW) License in North Dakota (or eligibility for licensure)
- Minimum 1 year of recent clinical social work experience
- Master's Degree in Social Work (MSW) from an accredited institution
- Current BLS Certification preferred
- Experience in hospital, acute care, behavioral health, or outpatient settings preferred
- Strong assessment, counseling, crisis intervention, and case management skills
- Excellent communication, interpersonal, and organizational abilities
- Experience using electronic medical record (EMR) systems preferred
- Ability to work independently while collaborating effectively with multidisciplinary teams
