Responsibilities include assessing social service needs of patients.
Providing psychotherapeutic intervention and counseling for individuals.
Assisting with outreach, program development, and education.
Assessing the psychosocial needs of individuals, partners, and families.
Conducting social work assessments and providing ongoing social work services.
Preparing comprehensive care plans and evaluating progress with clients.
Providing counseling for individuals, families, and groups; referring patients for psychiatric evaluation and treatment as needed.
Assessing, advising, advocating, and assisting patients in obtaining benefits.
Developing linkages between providers, social service agencies, and other resources to ensure continuity of care.
Identifying and addressing barriers to care and retention in the program.
Participating in case conferences with providers to ensure service coordination and continuity of care.
Participating in developing and presenting community outreach and education programs.
Providing counseling and testing services.
Advocating for clients across the spectrum of service providers.
Participating in staff and case conferences.
Participating in grant writing and fundraising efforts.
Participating in a continuous quality management program.
