The role focuses on assessing patient needs, assisting with care planning, coordinating community resources, and supporting patients and families throughout their treatment journey. This position does not involve clinical therapy or counseling.
The Social Worker will primarily support dialysis patients and may be required to travel or conduct telehealth visits for additional outpatient locations as needed.
